Driving Record Request Form

This form is used for EOU staff, faculty, students, and partners to request a driving record to operate any vehicle while conducting EOU business. Processing time varies depending on the state of issuance. You will be notified via email with the results of your driving record check. You are not authorized to operate any vehicle until you receive the results.
